This form is meant to order a portrait and send the digital photo with this form.
If you send us one image only, please do the following:
fill in this form
use the browse button of the last item of the form to find on your computer the image you want to send us
if you have filled in the form completely then send it to us with the submit button. If you send us a large image it may take a while for the form to proceed.
If the form was successfully submitted, you will see a 'thank you' page. If sending failed then you will see an error-page. In that case, please contact us.
Once the form is submitted, an email robot will send a confirmation to your email address. If you do not receive such a confirmation, you may have filled in a wrong email address. In that case, contact us.
Within a working day after you received the confirmation from the robot, we will contact you by email.
If you send us more than one image, send us the first one with this form as described above. Once you receive the confirmation of our robot then reply on that email and attach the other images.
